Wrexham Children’s Services are looking for an experienced Social Worker to join their established and high-performing Leaving Care Team, supporting care-experienced young people aged 15–25 as they transition towards independence.
This is an excellent opportunity for a committed practitioner who is passionate about relationship-based, trauma-informed practice and helping young people achieve positive outcomes.
Key Responsibilities
- Support care-experienced young people with transition and pathway planning.
- Complete LAC reviews and statutory visits.
- Promote independence, resilience and positive outcomes.
- Apply corporate parenting principles and trauma-informed approaches.
- Build strong, trusting relationships with young people.
- Work collaboratively with families, professionals and partner agencies.
- Model best practice and provide mentoring and support to colleagues.
- Maintain accurate case records and ensure statutory requirements are met.
- Apply relevant Welsh legislation, guidance and social care frameworks.
Essential Requirements
- Recognised Social Work qualification – essential.
- Experience working with children, young people or care-experienced young people.
- Experience completing LAC reviews and statutory visits.
- Good understanding of Leaving Care and transition services.
- Knowledge of relevant Welsh legislation and children’s social care requirements.
- Strong relationship-based and trauma-informed practice skills.
