About the Role
We are currently recruiting for an experienced Agency Social Worker to join the Team for Older People within Wrexham County Borough Council. Based at Crown Buildings, Wrexham, this is a varied hybrid role combining office-based working, community and hospital visits, and home-based working. You will manage a caseload of older adults, working collaboratively with citizens, families, carers and partner agencies to promote independence, maximise wellbeing and achieve positive outcomes. You will also participate in the team’s duty rota, supporting the management of new referrals, urgent enquiries and priority work.
Key Responsibilities
- Manage a caseload of older adults, providing high-quality strengths-based social work.
- Undertake comprehensive needs-led assessments and develop appropriate care and support plans.
- Review care and support arrangements to ensure they continue to meet the needs and desired outcomes of citizens.
- Complete Mental Capacity Assessments and Best Interest decisions in accordance with legislation and professional standards.
- Respond appropriately to adult safeguarding concerns.
