Join a Dynamic Discharge to Assess Team
We are seeking an experienced Occupational Therapist to join a busy Discharge to Assess (D2A) service based in Greater London. This is an excellent opportunity to work within a locality-based, multi-disciplinary team, supporting adults to achieve safe, timely hospital discharges while promoting independence within their own homes.
You will manage a varied caseload of approximately four cases per week, undertaking both standard and complex Occupational Therapy assessments, recommending equipment and adaptations, and working collaboratively with health and social care professionals to deliver outstanding, person-centred outcomes.
Key Responsibilities- Complete holistic, person-centred Occupational Therapy assessments within the community.
- Assess, recommend, and review both simple and complex equipment, major adaptations, and manual handling solutions.
- Develop support plans that maximise independence and reduce long-term care needs.
- Provide advice and training to service users, carers, and professionals on the safe use of equipment and moving and handling techniques.
- Work collaboratively within a multi-disciplinary team to facilitate safe and effective discharges.
