We are looking for an experienced Occupational Therapist to join the Community Occupational Therapy team in Ceredigion. This is an exciting opportunity to make a real difference by supporting adults and older people to maximise their independence, safety, and quality of life within their own homes and communities.
Working as part of a multidisciplinary team, you will manage a varied caseload, carrying out specialist occupational therapy assessments, recommending equipment and housing adaptations, completing moving and handling assessments, and delivering person-centred interventions that enable individuals to achieve their goals.
Key Responsibilities
- Manage a varied community caseload of adults and older people.
- Carry out comprehensive Occupational Therapy assessments and interventions.
- Assess and recommend specialist equipment and major/minor housing adaptations.
- Complete manual handling and risk assessments.
- Work collaboratively with health, social care, and partner agencies.
- Supervise Occupational Therapy Assistants and support Occupational Therapy students where appropriate.
- Maintain accurate records and deliver evidence-based, person-centred care.
Requirements
- Degree or Diploma in Occupational Therapy.
- Current HCPC registration.
