We are seeking an experienced Occupational Therapist to join a busy Discharge to Assess (D2A) service based in Greater London. This is an excellent opportunity to work within a locality-based, multi-disciplinary team, supporting adults to achieve safe, timely hospital discharges while promoting independence within their own homes.

You will manage a varied caseload of approximately four cases per week, undertaking both standard and complex Occupational Therapy assessments, recommending equipment and adaptations, and working collaboratively with health and social care professionals to deliver outstanding, person-centred outcomes.

Key Responsibilities
  • Complete holistic, person-centred Occupational Therapy assessments within the community.
  • Assess, recommend, and review both simple and complex equipment, major adaptations, and manual handling solutions.
  • Develop support plans that maximise independence and reduce long-term care needs.
  • Provide advice and training to service users, carers, and professionals on the safe use of equipment and moving and handling techniques.
  • Work collaboratively within a multi-disciplinary team to facilitate safe and effective hospital discharges.
  • Maintain accurate case records and ensure compliance with professional standards, legislation, and organisational policies.
  • Independently manage, prioritise, and progress a complex caseload.
Essential Requirements
  • HCPC registration as an Occupational Therapist.
  • Degree or Diploma in Occupational Therapy recognised by the HCPC.
  • Extensive experience assessing for and recommending specialist equipment, major adaptations, and complex manual handling solutions.
  • Strong knowledge of adult social care legislation, housing legislation, and relevant statutory frameworks.
  • Experience undertaking complex Occupational Therapy assessments, reviews, and support planning.
  • Ability to work autonomously while contributing effectively within a multi-disciplinary team.
  • Excellent assessment, communication, and report-writing skills.
