LinsCare is recruiting an experienced Senior Practitioner to join the Hospital Team supporting Richmond and Wandsworth.
This is an opportunity for a confident social work professional with strong hospital discharge experience to provide practice guidance, support colleagues with complex decisions and promote safe, timely discharge planning.
We are looking for:
- A qualified social worker registered with Social Work England
- Experience in hospital social work and discharge planning
- Strong knowledge of the Care Act, Mental Capacity Act and adult safeguarding
- Experience supporting colleagues and working with health and social care partners
Mandatory: Must have a UK Passport or a Full Right to Work in the UK
