We are looking for an experienced Adult Social Worker to join a Community Care Team in Liverpool, supporting vulnerable adults to live safely, independently and with dignity.
Key Responsibilities
- Complete Care Act 2014 assessments, reviews and support plans.
- Undertake mental capacity assessments and contribute to best-interest decisions.
- Respond to safeguarding concerns and undertake Section 42 enquiries.
- Develop strengths-based, personalised care and support plans.
- Assess and manage risks relating to abuse, neglect and vulnerability.
- Work closely with NHS services, Occupational Therapists, mental health teams, housing and voluntary organisations.
- Support adults, families and carers with information, advice and access to appropriate services.
- Maintain accurate case records and prepare reports for panels and legal proceedings.
- Promote independence, choice, wellbeing and positive outcomes.
Essential Requirements
- Recognised Social Work qualification (BA/BSc/MA or equivalent).
- Current Social Work England registration.
- Strong knowledge of the Care Act 2014, Mental Capacity Act 2005 and safeguarding procedures.
- Experience in adult social care, assessments, care planning and safeguarding.
- Enhanced DBS clearance.
- Full UK driving licence and access to a vehicle for community visits, where required.
