Lambeth Hospital Social Work Team is seeking an experienced Hospital Social Worker to support adults admitted to King's College Hospital and other hospitals within and outside the borough. This hybrid role requires 3 days on site in Lambeth with 2 days from home.
You will carry out Care Act assessments, contribute to discharge planning and work with MDT colleagues to promote safe, timely support for residents. The role offers a 6-month maternity cover and a pay rate of £39.87 per hour Umbrella.
