We are looking for an experienced Mental Health Social Worker to join a busy Adult Mental Health team in London. This is an excellent opportunity to work with adults with complex mental health needs, providing high-quality assessments, safeguarding interventions, and person-centred support to achieve positive outcomes.
Key Responsibilities
- Manage a caseload of adults with complex mental health needs.
- Carry out Care Act assessments, reviews, and support planning.
- Undertake access and assessment duties and act as a keyworker.
- Investigate safeguarding concerns and complete safeguarding enquiries.
- Prepare social circumstances reports and maintain accurate case records.
- Work collaboratively with multidisciplinary teams, families, carers, and partner agencies to deliver coordinated care.
Requirements
- Qualified Social Worker with current Social Work England registration.
- Experience working within Adult Mental Health services.
- Previous access and assessment and keyworker experience.
- Strong knowledge of the Care Act, safeguarding, and adult social care legislation.
- Excellent assessment, communication, and case management skills.
