Tripod are seeking an experienced Occupational Therapist to join the Reablement Team based at County Hall, Trowbridge. This opportunity has arisen due to increased service demand following the successful implementation of the Home First contract. This is a rewarding opportunity to support individuals to remain as independent as possible within their own homes and communities.
The Role
- Deliver strengths-based occupational therapy assessments
- Design and implement innovative reablement plans tailored to individual needs
- Provide professional advice on equipment, adaptations, and decision-making
- Support individuals to maximise independence and reduce long-term care needs
- Manage a varied caseload across community and home settings
- Work collaboratively with health and social care professionals
This role offers flexibility and autonomy in managing your workload, with a hybrid working arrangement:
- 50% of administration time completed in the office
- Remaining time split between home working and community visits
Requirements
- A recognised qualification in Occupational Therapy
- Minimum 2 years' post-qualification experience
- HCPC registration
- Experience within community or reablement settings preferred
- A valid full UK driving licence and access to transport
