We are currently recruiting for an experienced and dedicated healthcare professional to join a busy Intermediate Care service. The successful candidate must:

  • Be a car driver with access to a vehicle for community-based visits and service delivery
  • Hold current HCPC registration
  • Have experience working within a Multidisciplinary Team (MDT) environment
  • Have previous rehabilitation experience, which would be highly advantageous
  • Possess post-graduate clinical experience, as the service manages complex caseloads and requires confident clinical decision-making

This role involves working across all Intermediate Care pathways, including:

  • Reablement services
  • Rapid Response pathways
  • Rehabilitation beds and community rehabilitation services

Candidates must also demonstrate previous community experience and the ability to work independently within patients' homes and community settings while delivering high-quality, patient-centered care. The ideal applicant will be adaptable, organized, and capable of managing complex patient needs within fast-paced healthcare environments. Strong communication, assessment, rehabilitation, and care-planning skills are essential to support patient recovery, promote independence, and prevent unnecessary hospital admissions.
