We are looking for an enthusiastic and motivated ACP/ANP to join our team from July 2026. We are a friendly and supportive practice with a list size of approximately 10,500 patients, based 2.5 miles from Huddersfield town centre. We are a total triage practice and utilise EMIS, Patchs and AccuRx to support efficient working.
We are looking for an experienced ACP/ANP to join our team for 2 days per week onsite, with the option of up to an additional 1 day per week remote working (e.g. telephone/online consultations and admin), subject to service needs these sessions may be onsite. The role will include home visits and you must be an independent prescriber.
Main duties of the job:- Providing same day and routine face-to-face, telephone and online consultations for a range of undifferentiated and undiagnosed presentations.
- Assess, diagnose, treat and manage patients, including prescribing medications in line with national and local guidelines.
- Undertake home visits including care homes where required.
- Manage patients with long-term conditions.
- Identify and act on red flags and serious pathology, arranging urgent assessments or referrals as needed.
- Request and interpret appropriate investigations and follow up results, training will be provided as needed.
- Contribute to on-the-day demand management as part of a multidisciplinary team.
- Provide clear, accurate and contemporaneous clinical records.
- Participate in clinical meetings, MDTs and case discussions.
- Support the education of other staff where appropriate.

Our team consists of 5 GP partners, 3 salaried GP's, 2 nurses, 1 HCA, 1 GPA and 2 GP registrars as well as 10 admin staff.
