About Us

We are a small, friendly GP practice committed to delivering high-quality patient care to our local community. We are seeking an experienced and motivated Practice Nurse to join our team.

The Role

As an integral member of our clinical team, you will provide evidence-based nursing care and support a wide range of chronic disease management and preventative health services. You will work closely with GPs, healthcare assistants, pharmacists, and administrative staff to deliver outstanding patient-centred care.

Key Responsibilities

  • Chronic disease management including asthma, COPD, diabetes and cardiovascular disease reviews
  • Cervical screening and women’s health services
  • Childhood and adult immunisations and vaccinations
  • Travel health assessments and vaccinations
  • Wound care, dressings and suture removal
  • Health promotion and lifestyle advice
  • Phlebotomy, blood pressure monitoring and other clinical procedures
  • Supporting quality improvement initiatives and achievement of QOF targets
  • Maintaining accurate clinical records and adhering to professional standards

The successful candidate will

  • Be a registered nurse with current NMC registration
  • Have previous experience as a Practice Nurse working within primary care
  • Demonstrate competence in chronic disease management
  • Be experienced in cervical cytology and immunisations
  • Possess excellent communication and interpersonal skills
  • Be able to work independently whilst contributing effectively as part of a multidisciplinary team
  • Show a commitment to continuous professional development and high standards of patient care
  • Asthma and/or COPD diploma
  • Experience using System One
