Who You Are

We are looking for a Qualified Practitioner at level 1 or 2 with an enthusiastic and positive 'can do' approach. You must hold a Nursing or Social Work qualification (DIPSW, CQSW, CETSW, CSS or BA/Bsc / Social Work) and have current registration with the NMC or Social Work England. You should have a strong commitment to continuous professional development and working as part of a team. Sound knowledge of legislation underpinning statutory Adult Care Practice is essential, along with values promoting wellbeing and empowerment.

What the Job Involves

Join our Intake and Transitions Team on a full-time, one-year contract. You will work within the Learning Disability Service to support transitions for young people moving from Children's to Adult Services across Lincolnshire. Your role will involve enabling young people to make informed choices and achieve the best outcomes during their transition to adulthood. By working in partnership with Children's Services and care and health partners, you will link with local schools, attend EHC reviews, and provide support to young people, families, and colleagues in educational establishments.
