LADMS seeks a highly motivated and experienced Advanced Nurse Practitioner (ANP) or Advanced Clinical Practitioner (ACP) to join our dynamic and patient-focused team. This exciting role involves delivering high-quality, evidence-based care, working autonomously while collaborating closely with multidisciplinary colleagues. The successful candidate will be committed to clinical excellence, innovation, and improving patient outcomes within a supportive and forward-thinking organisation.
Main duties of the job
- Work autonomously in primary care settings, community clinics, private healthcare settings, and remotely where appropriate.
- Prescribe appropriately and adhere to all guidelines.
Job responsibilities
- Demonstrate expert clinical knowledge and skills, making autonomous decisions in the assessment, diagnosis, and treatment of patients.
- Manage patients presenting with a range of acute and chronic medical conditions, providing subject-matter expert advice.
- Implement and evaluate individual specialised treatment plans for chronic disease patients.
- Identify, manage, and support patients at risk of developing long-term conditions.
- Actively engage in peer review to inform practice and implement strategies for improvement.
- Maintain accurate clinical records in accordance with legislation.
- Develop and implement robust governance systems and contribute to practice targets.
- Support the team in dealing with clinical emergencies and safeguarding matters.
- Deliver opportunistic health promotion where appropriate.
Secondary responsibilities
- Support the IPC lead, ensuring compliance, and undertake audit as necessary.
- Participate in local initiatives to enhance service delivery and patient care.
- Develop an area of specialist interest, taking the lead within the organisation.
Person Specification
Qualifications
- Educated to Masters degree within the HEE document titled Multi-professional framework for advanced clinical practice in England.
- Qualified independent prescriber – V300.
- Registered on the appropriate professional register.
- Teaching qualification.
- ALS and PALS.
Experience
- Experience within the four pillars of advanced clinical practice.
- Experience of prescribing and undertaking medication reviews.
- Experience of working in a primary care environment.
Clinical knowledge and skills & Other Requirements
- Good understanding of national guidelines for ILD, COPD, asthma, and oxygen.
- Ability to record accurate clinical notes.
- Polite, confident, flexible and cooperative.
- High levels of integrity and loyalty.
- Enhanced Disclosure Barring Service (DBS) check.
- Occupational health clearance.
- Access to own transport and ability to travel across locality regularly.
