LADMS seeks a highly motivated and experienced Advanced Nurse Practitioner (ANP) or Advanced Clinical Practitioner (ACP) to join our dynamic and patient-focused team. This exciting role involves delivering high-quality, evidence-based care, working autonomously while collaborating closely with multidisciplinary colleagues. Main duties of the job

ANP/ACP will work autonomously in primary care settings, community clinics, private healthcare settings, and remotely where appropriate. The role requires working in busy, thriving healthcare environments across a multitude of services and sites, prescribing appropriately and adhering to all guidelines.

Job responsibilities

The Advanced Clinical Practitioner (ACP) has been assessed as competent in practice using their expert clinical knowledge and skills, and has the freedom and authority to act and make autonomous decisions in the assessment, diagnosis, and treatment of patients.

The post holder works at the level of advanced clinical practice, demonstrating the underpinning competencies applicable to the specialty or subject area, including knowledge, skills, and behaviours appropriate to the HCP setting and job role. An ACP is autonomous in decision‑making based on assessment, diagnosis, and interpretation of test results and initiates, evaluates, and modifies a range of interventions which may include prescribing medicine, therapies, lifestyle advice, and care.

  • Practice in compliance with professional conduct and within scope of practice, being responsible and accountable for decisions, actions, and omissions.
  • Demonstrate a critical understanding of broadened responsibility, autonomy, and limits of competence, especially with complexity, risk, and incomplete information.

  • Implement and evaluate individual specialised treatment plans for chronic disease patients.
  • Identify, manage, and support patients at risk of developing long‑term conditions, preventing adverse effects to health.
  • Be a professional role model for the organisation.
  • Actively engage in peer review to inform own and others’ practice, formulating and implementing strategies to act on learning and make improvements.
  • Actively seek feedback and involve individuals, families, carers, communities, and colleagues in co‑production of service improvements.
  • Critically assess and address own learning needs, negotiating a personal development plan reflecting the breadth of ongoing professional development across the four pillars of advanced clinical practice.
  • Provide chronic disease clinics, delivering patient care as necessary, and refer patients to secondary/specialist care as required.
  • Maintain accurate clinical records in accordance with legislation and ensure SNOMED CT codes are used effectively.
  • Develop, implement, and embed an effective call/recall system.
  • Critically appraise and synthesize the outcome of relevant research, evaluation, and audit, using results to underpin own practice and inform others.
  • Develop and implement robust governance systems and contribute to practice targets (QOF, etc.), complying with local and regional guidance.
  • Chaperone patients where necessary.
  • Support the team in dealing with clinical emergencies.
  • Support the team with all safeguarding matters in accordance with local and national policies.
  • Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
  • Deliver opportunistic health promotion where appropriate.

Secondary responsibilities

  • Support the IPC lead, ensuring compliance, and undertake audit as necessary.
  • Monitor and ensure the safe storage, rotation, and disposal of medicines.
  • Undertake overall management of a clinical team, providing guidance when necessary, and act as a mentor to students and newly qualified staff.
  • Maintain an effective clinical staff rota ensuring all clinics are staffed appropriately.
  • Participate in local initiatives to enhance service delivery and patient care.
  • Support and participate in shared learning within the organisation.
  • Develop an area of specialist interest, taking the lead within the organisation.
  • Continually review clinical practices, responding to national policies and initiatives where appropriate.
  • Participate in the review of significant and near‑miss events using a structured approach (root cause analysis).
  • Support the development of clinical services within the organisation, liaising with external agencies and professional organisations as required.
  • Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation.

Person Specification

Qualifications

  • Educated to Masters degree within the HEE document titled Multi‑professional framework for advanced clinical practice in England.
  • Qualified independent prescriber – V300.
  • Registered on the appropriate professional register.
  • Teaching qualification.
  • ALS and PALS.

Experience

  • Experience within the four pillars of advanced clinical practice.
  • Job plan demonstrating advanced clinical practice and equity with peers at this level.
  • Experience of prescribing and undertaking medication reviews.
  • Experience of working in a primary care environment.

Clinical knowledge and skills & Other Requirements

  • Clinical knowledge and skills – good understanding of national guidelines for ILD, COPD, asthma, and oxygen, including diagnosis and evidence‑based interventions.
  • Assist in the care of respiratory patients including assessment, care planning, and ongoing support, monitoring adherence, inhaler technique, and side effects.
  • Understand the importance of evidence‑based practice.
  • Broad knowledge of clinical governance.
  • Ability to record accurate clinical notes.
  • Ability to work within own scope of practice and understand when to refer to GPs.
  • Knowledge of health promotion strategies.
  • Understand the requirement for PGDs and associated policy.
  • Polite, confident, flexible and cooperative.
  • Motivated, forward‑thinking.
  • Problem‑solver with the ability to process information accurately and effectively, interpreting data as required.
  • High levels of integrity and loyalty.
  • Sensitive and empathetic in distressing situations as well as ability to work under pressure.
  • Knowledge of public health issues in the local area.
  • Awareness of issues within the wider health arena.
  • Enhanced Disclosure Barring Service (DBS) check.
  • Occupational health clearance.
  • Meet the requirements and produce evidence for revalidation.
  • Evidence of continuing professional development (CPD) commensurate with the role of an ACP.
  • Access to own transport and ability to travel across locality regularly.
  • Flexibility to work outside core office hours.
