About the role
Within the role of a Reconnect Navigator, you will be required to work with individuals who experience poor mental health, learning disability, substance misuse or other psychosocial vulnerabilities to enable them to identify, implement and maintain engagement in services and positive lifestyle changes which will support their ability to engage with services to support and address their needs.
The key responsibilities and tasks within this role are:
Engage with individuals to understand barriers to engagement and change
- Understand barriers to engagement and to identify strategies for overcoming these barriers.
- Assess individual and wider needs and identify ways to manage, change, reduce or eliminate difficulties which are discouraging positive change, for example health, mental health, accommodation, finance, gambling or relationship concerns.
- Identify ways for individuals to engage with positive change lifestyle changes such as family or community integration, employment, training or education
Identify and work with relevant agencies and networks to ensure that the best support is in place
- Research and identify the agencies and support networks which are most appropriate and most likely to enable the individual to make positive health and wider lifestyle changes
- Work with the individual and the relevant networks to encourage ongoing positive engagement and support
- Engage with prisons and probation services where relevant and attend meetings, i.e resettlement boards and other meetings in the community and custody
Work with and support individuals to make positive lifestyle changes
For example by:
- Helping individuals to engage and sustain engagement with agencies and networks which would provide the best help and support
- Assisting individuals to communicate and work with organisations which could affect their day-to-day life such as health / mental health providers, financial institutions, education and training establishments, employment agencies
- Support individuals to build positive relationships with the local community, friends and family
- Encouraging and helping to enable positive lifestyle changes and a new vision of the future
Adhere to relevant policies, procedures and record-keeping requirements
- To complete required funder reports to demonstrate activity and outcomes
- Ensure that prison and LAT case management systems are updated where appropriate
- Keep all records relating to the individual and the contract accurate and up to date and be able to produce all relevant information when required
- Engage fully in supervision meetings and keep your line manager properly informed of all important information pertaining to the individual cases which you are responsible for
- Ensure that all relevant LAT policies, procedures and Matrix Quality Standards are adhered to
Undertake any other duties within the overall scope of the job when required.

This role requires the post holder to have an NVQ Level 4 in Information, Advice and Guidance. Applicants that do not currently have this qualification should not be discouraged from applying, as LAT are committed to supporting the successful candidate to complete this qualification following a successful 5 month probation period.

Salary:
- £24,454 – Trainee salary band for those with no relevant experience or qualification
- £25,180– Middle band salary for those with some relevant experience or a relevant level 3 qualification
- £28,499 – Level 4 qualified band for those with NVQ4 in IAG or relevant experience
LAT will determine salary level at the point of offer, and the justification for this salary will be provided to you at the point of offer. LAT will support those categorised at trainee or middle band salary to complete their NVQ4 in IAG. Following completion of this qualification, salary will increase to level 4 qualified band. Staff members that are supported to complete their NVQ 4 in Information, Advice and Guidance will enter into a study agreement for this qualification. Unsuccessful candidates will be informed that they have not been selected for a initial telephone screening interview. Candidates that are successfully shortlisted will be contacted by the hiring manager for an initial telephone screening interview on the date listed above. If you are successful following the initial telephone screening interview, you will be contacted to confirm your interview slot for a panel interview on the date listed above. If you have not been successful after your telephone interview, this will be confirmed by email.
Successful candidates will be sent an invitation to attend a panel interview via email, which will contain the details of the interview.
Lincolnshire Action Trust is a ‘Ban the Box’ employer, this means we do not ask for details about previous criminal convictions at application stage but instead ask this at interview stage. Therefore, during the interview we will ask the following questions in line with our Safer Recruitment Procedures:
- Do you have any unspent conditional cautions or convictions under the Rehabilitation of Offenders Act 1974?
- Do you have any adult cautions (simple or conditional) or spent convictions that are not protected as defined by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020?
It is important that you provide accurate and factual information about any previous convictions, including offence, date of conviction and disposal. This includes both spent and unspent convictions.
Safer Recruitment and Role Requirements
Vetting:
Successful applicants will be required to gain successful vetting clearance. This will be applied for by LAT on your behalf, and you will be required to comply with all processes involved with this such as verifying documents and completing questionnaires in a timely manner. Because of this requirement, all candidates must be 18+. There is no cost to you to undergo this vetting.
Candidates should discuss any potential concerns about vetting during interview, as failure to do so may result in delays to starting in post. Failure to be granted vetting clearance is likely to result in our offer of employment being withdrawn.
DBS:
Successful applicants will be required to obtain an enhanced DBS with Adult barred list. This will also be applied for by LAT, via an external organisation. This process will be discussed further at the point of applying for the DBS. There is no cost to you to undertake a DBS.
Any concerns about information that may be contained on your DBS certificate should be discussed with the panel during the interview, as information on a DBS such as previous convictions etc may result in our offer being withdrawn, however this will be reviewed on a case by case basis.
Other Safer Recruitment checks such as references covering a three year continuous period will also be required, alongside any additional checks that are deemed necessary such as social media and internet searches.
Other useful information
Our Induction period will be predominantly based at our Head Office in Lincoln, alongside other training sessions that you may need to attend throughout your employment, and therefore the ability to travel to Lincoln is necessary.
Where your work location is not our Head Office, expenses will be paid to you for your travel there.
The successful candidate will be subject to a 5 month probationary period, in which your suitability for the role will be reviewed regularly.
