About the Practice
We are a busy, innovative NHS GP practice in the heart of Old Trafford serving around 9,000 patients across M15 and M16. Our community is diverse, dynamic and complex. We offer services in over 70 languages and are deeply committed to reducing health inequalities in one of Greater Manchester’s most vibrant areas. The practice, Limelight Health and Wellbeing Hub, is located at 1 St Brides Way, Manchester (M16 9NW). We provide comprehensive NHS primary care services to patients in the M15 and M16 postcode areas and are proud members of the North Trafford Primary Care Network (PCN).

Main Duties of the Job
Working four days a week (37.5 hours), you will practise autonomously across a broad range of settings, including face‑to‑face, telephone and digital triage consultations, home visits for housebound and high‑risk patients, ordering and reviewing investigations (bloods, imaging and ECGs), supporting the clinical and admin teams to deliver seamless patient care, conducting clinical audits and driving quality improvement, and contributing to CQC readiness and clinical governance.
Job Responsibilities
We expect you to perform the following responsibilities:
- Conduct face‑to‑face consultations, assessing, diagnosing, and managing undifferentiated and complex presentations across all age groups.
- Undertake home visits for housebound, frail, and high‑risk patients, providing comprehensive clinical assessment and management in the patient’s own environment.
- Deliver telephone and digital triage, prioritising patient need and directing to the most appropriate care pathway in a timely and clinically safe manner.
- Order, review and act upon a full range of investigations including blood tests, imaging, ECGs and other diagnostics, ensuring results are communicated promptly and appropriately.
- Prescribe medications and treatments within the practitioner’s scope of practice, in line with current clinical guidelines and the practice prescribing formulary (where prescribing qualification is held).
- Manage and review patients with long‑term conditions, contributing to the practice’s chronic disease management programmes.
- Refer patients to secondary care, specialist services and community teams as clinically indicated.
Team and Multidisciplinary Working:
- Work collaboratively with GPs, nurses, healthcare assistants, pharmacists, social prescribers, and other clinical and non‑clinical staff to ensure seamless, coordinated patient care.
- Support and mentor junior clinical colleagues, including students, trainees and health‑care assistants, promoting a positive learning environment.
- Contribute to multi‑disciplinary team (MDT) meetings, clinical case discussions and practice clinical meetings.
- Liaise effectively with secondary care providers, community health teams, social care and wider PCN colleagues to support care transitions and complex case management.
Working with the Admin Team:
- Work in close partnership with the practice administration team to ensure efficient and patient‑centred appointment management, workflow and communication.
- Support administrative processes including clinical coding, referral completion and accurate maintenance of electronic patient records using the clinical system EMIS/Docman.
- Contribute to the management of incoming correspondence, clinical letters and test results in a timely manner, working within agreed protocols.
- Provide clinical guidance to admin and reception staff to support accurate sign‑posting, call handling and care navigation.
Quality Improvement and Audit:
- Conduct clinical quality audits, including significant event analysis, clinical audits and patient satisfaction reviews, using findings to drive measurable improvements in care.
- Participate in and lead quality improvement (QI) projects aligned to the practice’s development priorities and the wider PCN quality agenda.
- Support the practice’s compliance with CQC standards and NHS contractual requirements, contributing to evidence gathering and quality assurance processes.
- Monitor and review clinical outcomes, identifying trends and areas for improvement in patient safety and care quality.
- Engage with the practice’s clinical governance framework, contributing to significant event reviews, safety huddles and learning from complaints or incidents.
Professional Standards and Development:
- Maintain full registration with the relevant regulatory body (NMC for nurses, HCPC for paramedics) and adhere to the relevant professional Code of Conduct at all times.
- Maintain a current and valid portfolio of evidence aligned to the Royal College of Emergency Medicine (RCEM) or equivalent ACP framework, demonstrating ongoing competence at advanced level.
- Engage in regular clinical supervision, annual appraisal and continuing professional development (CPD), maintaining up‑to‑date knowledge and skills.
- Ensure mandatory training, including safeguarding, infection prevention and information governance, is completed within required timescales.
- Act as a positive ambassador for the practice and the wider PCN, upholding the values and ethos of Limelight Health and Wellbeing Hub at all times.
