We are currently seeking a dedicated care plan coordinator to join our compassionate team in Cheltenham and Stroud. In this crucial role, you will be responsible for ensuring that every care home resident has a personalised care plan that accurately reflects their individual needs, preferences, and goals. You will work closely with residents, families, and the healthcare team to ensure the highest standards of care are maintained. Key Care Coordinator Responsibilities
- Creating detailed, person-centred care plans together with care home residents
- Coordinating with other professionals involved in residents’ care, where necessary
- Ensuring care plans are kept up-to-date
- Supporting the care team to effectively execute care plans
- Providing the care team with necessary training
- Documenting the wellbeing of residents
Required Skills & Qualifications
- A degree in nursing, social care or a related field
- Experience in a care home setting – desirable but not essential
- Thorough understanding of principles and legislation underpinning elderly care
- Great communication skills, both written and oral, in order to build positive relationships with care home residents, staff and residents’ family members, and other healthcare professionals
- Time management to coordinate and organise tasks in your care coordination tasks
- Ability to work well as part of a team
- Attention to detail, ensuring care plans are accurate and truly reflect the needs and wishes of the residents
- A valid driver’s licence (for community-based roles)
