Community Advantage Rehabilitation (CAR) – a division of Lifemark is looking for Registered Occupational Therapists to provide services to our adult clients within the community. The position can be flexible, with working hours ranging from part-time to full-time depending on your desired client caseload size.

As Durham Region’s largest community-based rehabilitative therapy provider, we have been providing community rehabilitation services for adults and children since 1997. Our vision is to be recognised as a trusted and quality provider of healthcare services, helping adults and children to maximise their functional abilities within their environment and enable them to participate more fully in life’s activities and rewards.

OT Responsibilities:

  • Provide ministry funded, community-based Occupational Therapy services within the client’s place of residence and/or through virtual services
  • Provide occupational therapy assessment (through interview, standardized and informal assessment techniques) in the areas of home safety, home modifications, equipment prescription, fall prevention, mobility, cognition, stroke, etc.
  • Develop and implement best practice and evidence-based occupational therapy intervention in collaboration with clients/caregivers
  • Assign patient care plan, and provide supervision and monitoring of occupational therapy assistants when appropriate
  • Effectively communicate information by providing education to client/caregivers and linking to community agencies/resources when appropriate
  • Complete funding applications as applicable
  • Complete and maintain current and accurate documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and COTO standards
  • Opportunity to service private clients

Requirements:

  • Registered Occupational Therapist with the College of Occupational Therapists of Ontario (required)
  • Experience working within a community setting is an asset but new graduates are welcome
  • Ability to work independently and as part of an inter-professional team
  • Ability to prioritise, manage self-scheduling and meet reporting deadlines
  • Excellent oral, written and report writing skills
  • Proficiency with computers
  • Strong interpersonal and organisational skills
  • Reliable transportation is required for this position
