We are seeking an enthusiastic, reliable, highly motivated individual to the role of Diabetic Specialist Nurse. You will be joining a well-established team in a well-established practice. The ideal candidate will be an experienced Practice Nurse with exceptional diabetic knowledge, or a Diabetic Specialist Nurse who has previously worked in secondary care services.
Main duties of the job:
- Deliver high-quality, evidence-based care to patients with diabetes in a primary care setting
- Run autonomous nurse-led diabetes clinics, including assessment, diagnosis support, and ongoing management
- Initiate, review, and optimise diabetes treatment plans, including insulin and other injectable therapies
- Provide structured education and self-management support to patients and carers
- Monitor and manage long-term conditions, including complications and comorbidities associated with diabetes
- Interpret clinical data (e.g. HbA1c, blood glucose readings, lipid profiles) and act accordingly
- Undertake annual diabetes reviews and contribute to QOF targets and clinical audits
- Work collaboratively with GPs, practice nurses, HCAs, and the wider multidisciplinary team
- Liaise with secondary care and specialist services where appropriate
- Promote health education, prevention, and lifestyle interventions
- Maintain accurate, contemporaneous clinical records in line with professional and legal standards
- Support the development and implementation of practice protocols and guidelines for diabetes care
- Contribute to staff training and act as a resource for diabetes knowledge within the practice
- Maintain up-to-date professional knowledge and participate in continuing professional development
