The Support Worker is an experienced professional dedicated to the support services for individuals in need. Key Responsibilities:
- Nurturing: Inspire and motivate residents to help them achieve their long-term life goals.
- Care Planning: Develop and implement personalised care plans for individuals under support.
- Assessment: Conduct thorough assessments of individuals' needs and adapt care plans accordingly.
- Guidance: Provide guidance and support to residents, ensuring adherence to house rules.
- Coordination: Collaborate with external agencies and healthcare professionals for holistic support.
- Compliance: Ensure strict adherence to relevant regulations and standards.
- Training: Participate in and facilitate training and development programs for continuous improvement.
Main Duties & Responsibilities:
- To undertake a mentoring role in ensuring the care and welfare of each resident in placement.
- To give guidance and support to all residents.
- To assist the Senior Support Worker with daily residential tasks.
- To provide key work sessions every week (minimum) with all residents.
- Ensure the care/placement plan for each young person is always followed and adhered to.
- To ensure education is promoted, supported and that each resident’s attendance in education is maintained.
- To ensure that all staff act in an appropriate manner.
- To ensure that the home's policies and procedures are always adhered to.
- To ensure all risk assessments are consistently and robustly applied.
- To ensure that all safeguarding concerns are reported to the Designated Safeguarding Lead/Head of Operation immediately.
- To read and familiarise with all the daily logs once on shift.
- To attend meetings and training as required.
- Undertake any other reasonable tasks deemed required as directed by the Senior Support Worker or Management.
At least one year of experience working in a residential setting with young people with complex and challenging behaviour.
Current Driving Licence.
Ability to multi-task and meet deadlines.
Education, Training and Qualifications:
- Any health care related NVQ Level 2 and above.
- Relevant health care short courses.
- Be prepared to complete training courses.
