Rehabilitation Team Lead in Southampton

Hours: 18 hours per week (6 Hours Team Leader duties remote; 12 hours Rehab Assistant duties Monday 10:00 – 16:00; Friday 10:00 – 16:00 – Can be flexible on days for the right candidate)

About the Role

You will be supporting a client with a Traumatic Brain Injury to achieve the highest level of independence in everyday functional tasks, and encourage active participation in goal-directed rehabilitation interventions. This includes assisting with functional daily living activities, supporting with organisation and planning, communication needs and providing emotional support. This client lives independently with their cat, enjoys community volunteering and creative hobbies and spending time with friends and family.

You will also undertake leadership responsibilities, including supervising the other Rehab Assistant, updating the rehabilitation plan and risk assessments, managing the rota and client schedule, liaising with the multi-disciplinary team and client’s family.

What We're Looking For

Due to the client’s need, we’re seeking a rehab assistant with:

• Experience working with clients with brain injury, neurological condition or mental health
• Comfortable working independently in community/home settings, with strong time management
• Some prior experience mentoring, supervising, or informally leading others (or a clear readiness to step into this)
• Excellent communication skills with client, their family, and the wider clinical team
• A full, valid driving licence and access to a vehicle
• Good IT skills, including competent in using Word and Excel
• Enhanced DBS check (or willingness to undergo one)
• Comfortable around animals, including cats and dogs
• Comfortable going swimming with the client as part of physio/leisure activities

Key Responsibilities

Direct Support

• Support client with cognitive rehabilitation, communication strategies, and activities of daily living
• Provide daily emotional support

Team leadership

• To actas a point of contactfor themembers of the Multi-Disciplinary Team (MDT) and family members
• To overseeintroduction andfollowingof therapyprogrammesand ensureRehab Assistants arefollowingandcompletedall designated plans and tasks.
• Provide supervision to Rehab Assistants
• Provide feedback on session notes and Identifyany Rehab Assistant training needs
• Organiseand lead RA meeting, create agenda, invite relevant MDT members
• To plan and co-ordinate appointments,eventsand tasks
• To carry out background research into local vocational, recreational and leisure opportunities and present findings to wider professional team
• Oversee medication collection,liaisingwith GP/pharmacist. Ensuring appointments are booked and chase up test results.
• To follow up on any incidents and near misses and carry out investigations as and whenrequired.
• Toact as a point of contact for the rehabilitation assistants to support and co-ordinate dealing and actioningpost/email correspondencein a timely manner
