Location: South

About the Opportunity:
Libertatem Healthcare Group, part of the Sonderwell Group is committed to providing high-quality, compassionate and person focused healthcare to our clients. We are looking for a passionate and dedicated Field Care Manager to oversee the delivery of a high standard of home care services. Your responsibilities will include conducting client assessments and managing care plans, ensuring our services meet compliance with CIW regulations. The role involves regular and frequent travel to client’s homes and could include some evenings and weekends, covering our South region including Somerset, Dorset & Hampshire.
Key Responsibilities:
- Care Planning & Assessment: Develop and review personalised care plans.
- Liaise with the operational team regarding performance issues and training needs.
- Compliance & Quality Assurance: Ensure services meet CIW standards, conduct audits, and maintain accurate records.
- Build relationships with clients and their families, address concerns, and update their care needs.
- Collaborate with healthcare professionals to ensure holistic care.
Requirements:
- NVQ Level 3 or 4 in Health & Social Care (or equivalent).
- Experience in a supervisory/management role within health and social care.
- A passion for delivering high quality care, exceptional communication and organisational skills.
- Full UK driving license and ability to travel using own car.
- This role requires travel around the South Wales region.
Benefits:
- A competitive salary of £27,000-£35,000 depending on skills and experience.
- Field-based role, working in our Swansea office around visits.
- Working hours – 9am-5:30 pm Monday to Friday with some flexibility over weekends/evenings.
- Mileage paid at 45p per mile.
- Annual leave - 25 days plus bank holidays.
- Mobile phone.
