About Us
Liaison Care, part of the Liaison Group, partners with Integrated Care Boards (ICBs) and Health Boards nationwide to improve service delivery within the NHS Continuing Healthcare Framework.
Job Responsibilities
- Lead Multi‑Disciplinary Team (MDT) meetings.
- Complete CHC Decision Support Tool or Adult Needs Assessment with eligibility recommendations.
- Conduct CHC reviews.
- Manage follow‑up calls and request additional documentation and evidence from professionals and providers.
- Assess and develop care and support plans in line with Liaison Care and ICB KPIs.
Qualifications
- NMC Registered Nurse.
- Minimum 12 months post‑qualification experience.
- Experience in NHS Continuing Health Team or community CHC assessments.
- Experience chairing MDT meetings.
- Ability to produce clear, comprehensive reviews and assessments.
- Knowledge of CHC Framework 2022, Care Act 2014 and Mental Capacity Act 2005.
- Computer literacy – Microsoft Office (Outlook, Excel).
Contract & Working Pattern
Permanent. Full‑time or part‑time, home or remote working, with term‑time hours.
Salary
Pay depends on experience.

Applicants must succeed in a DBS check under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
