About the role
As the first point of contact for patients, families, providers, and social workers, the Pathway Coordinatorwill play a key role in delivering a professional and responsive service. You’ll work independently with supervisory support, managing bookings, verifying data, and ensuring assessments are completed efficiently and effectively.

Key responsibilities
Key responsibilities include:
- Manage incoming case data in line with Liaison Care’s governance standards and protocols.
- Book clinical reviews, DSTs, and commissioning reviews; liaise with families, providers, and local authority partners to confirm assessment dates.
- Provide necessary paperwork to all parties; liaise with clients to obtain or share clinical documentation.
- Complete all booking-related admin tasks; ensure clinicians have required DSTs or review meetings scheduled weekly.
- Handle calls professionally and sensitively, especially with vulnerable individuals.
- Record cancellations and validate all work on Flo before submission, ensuring accuracy in formatting, spelling, names, and dates.
- Support clinical and commissioning teams with tasks and attend weekly team meetings to stay aligned with upcoming work.

- Experience using a Management Information System

- Experience in CHC or assessment work

- Ability to deliver work to tight deadlines
Qualifications & Technical Skills
- 5 GCSEs (or equivalent) including English and Maths
- High standard of written communication
- Ability to use performance software
- Experience in clinical booking
- Experience in Continuing Healthcare (CHC)
- Familiarity with FLO Management Information System
- Interest in learning new skills
