Responsibilities The Speech Therapist in Home Health is responsible for assessing and evaluating patients\’ needs related to speech and language disorders, as well as providing functional training in communication, swallowing, and cognitive impairments. Provide services within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team. Evaluate patients\’ functional abilities using diagnostic and prognostic tests and assist physicians in developing the therapy plan of care. Treat patients to improve effective communication based on their condition, following accepted standards of practice. Instruct patients, families, caregivers, and healthcare team members in areas related to speech therapy participation.
