Responsibilities
The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist develops a treatment plan, performs interventions to improve and enhance the patient's well-being, and evaluates progress. Evaluates the patient's functional status and needs, assisting the physician in developing the therapy plan of care. Observes and reports the patient's response to treatment and any changes to supervising nurse and/or physician. Treats patients to relieve pain, restore or develop function, and maintain maximum functioning according to standards of practice. Instructs and informs the healthcare team, patient, and family/caregivers about the plan of care, functional limitations, and progress toward goals.
Education and Experience
Licensure Requirements: Current Physical Therapy licensure in Illinois. Current CPR certification required. Must have a valid driver's license, vehicle insurance, and access to a dependable vehicle.
