We are looking for an experienced Senior Social Worker, preferably with Front Door or Hospital Discharge experience, to join our busy Front Door Service. The ideal candidate should have a strong background in managing staff, demonstrate expertise in risk management and safeguarding, and possess a commitment to promoting the principles of self-directed support and personalization in the care of vulnerable adults.

The role involves working as a senior case management social worker with a caseload and supervisory responsibility for a group of staff within a large team. Key responsibilities include:
