Lewisham and Greenwich NHS Trust is seeking a Clinical Primary Care Pharmacist to join the LIMOS service across community and interface settings. This Band 7, 22.5 hours per week post is a 12-month maternity cover based at University Hospital Lewisham and local community sites.
You will perform patient-centred medication reviews, provide prescribing support and work within an MDT to optimise medicines for older adults and those with complex needs.
