Job Description Summary (Main duties and responsibilities)
As a Specialist Clinical Pharmacist, the post holder will play a pivotal role within the pharmacy team, contributing to the delivery of a safe, effective, and patient-centred pharmacy service across the Trust.
Key Responsibilities include:
- Clinical Ward-Based Services: Delivering a high-quality, patient-focused clinical pharmacy service on wards, ensuring the safe and effective use of medicines.
- Supporting Timely and Safe Discharges: Facilitating early discharge and reducing hospital length of stay by:
- Optimising patients' drug therapy.
- Providing tailored education on medicine use, including support with self-medication and assessing the need for adherence aids.
- Offering comprehensive counselling to patients about their medications prior to leave and discharge.
- Ensuring continuity of care through robust discharge planning and communication with community healthcare providers.
