Overview

Band 6 – Outreach Mental Health Practitioner; Adult MH Planned Care Community Services; Homeless Mental Health Service (HMHS). Base: OSL House / Agile working. 1 x Full Time 37.5 Hrs per week.

An exciting opportunity has arisen for a compassionate, enthusiastic, motivated, hardworking individual with a mental health or Learning disability nursing, mental health occupational therapy or social work professional background to join the Homeless Mental Health service. This post involves providing engagement, assessment, support and access to treatment services for people who are homeless. The role includes street outreach to people who are rough sleeping, alongside the Council housing outreach teams. It can involve significant walking, working in inclement weather, challenging environments and occasional limited out of hours working (6am start). Applicants must be prepared to undertake these duties as part of HMHS.

This post will require working primarily across Charnwood, NW Leicestershire, Hinckley & Blaby districts along with city centre clinics and street outreach, so ideally would suit someone living in the west / North West of Leicester / Leicestershire to reduce home to work travel time and associated costs. Operating from various locations and some home working. The core pattern is Monday to Friday 8.30am to 4.30pm, with some occasional out of hours work (enhancements paid). Mileage is calculated from the nearest LPT premises to staff home address when travelling to visits; mileage rates will be payable from this designated location. A salary sacrifice or lease car scheme is in operation and, although some public transport may be required, applicants must be car drivers & owners (or have a suitable alternative).

Job Description Summary (Main Duties And Responsibilities)

The team is an active partner within the wider multi-agency statutory & voluntary sector homeless services within Leicester City. The successful applicant would help facilitate this partnership, as well as assist in developing the service and similar partnerships across the wider counties of Leicestershire & Rutland. As the service develops across these districts, work will be allocated around existing staff locations to reduce mileage and travel time; however, on occasion cover may be required for colleagues in other areas.
Candidates will need to demonstrate the ability to provide care for people presenting with mental health difficulties within a variety of settings including day centres, hostels & temporary accommodation and general public settings (street outreach). Excellent assessment, risk management, organisational and communication skills are essential, with a good working knowledge of community care & hospital admission & discharge processes. The ability to use initiative, work autonomously and manage time effectively is required, along with the ability to work as part of a team and wider multi-agency homeless services. Shadowing current MHPs is possible with prior arrangement.

Qualifications

Applicants should have a background in mental health or Learning disability nursing, mental health occupational therapy or social work.
