Job Description Summary (Main Duties And Responsibilities)
- To dispense prescriptions and final check prescriptions
- To carry out the distribution and stock control of medicines
- To visit designated wards and departments and to liaise with medical and nursing staff in relation to the stock control and quality control of medicines
- To liaise with ward staff and Pharmacists
- To screen patients on admission to the Trust, contacting GPs and / or other healthcare providers to ascertain the patient’s medication history and advising Trust staff of identified discrepancies.
- Screening patient’s own medicines brought into hospital for potential re-use.
- To identify potential pharmaceutical issues that may arise during discharge planning and work with multidisciplinary to resolve.
- To support the efficient procurement of all medicines used within the Trust and ensure that all medicines are procured in line with Trust & departmental policies & national frameworks.
- To contribute to, the management of short-term supplier ‘to-follows’ and longer-term medication shortages; and manage the application of these to ensure continuity of supply or sourcing of alternative treatments, escalating to the Lead Pharmacists or Pharmacy Operations Manager as appropriate.
