We are seeking to appoint an enthusiastic Community Hub Coordinator to support our teams and service users, within Community Health Services for Nursing and Therapy based principally at Coalville Health Centre in the Community Nursing office (free parking is available and this base is close to public transport). Occasional travel will be required to the base at Legion House, in Ashby-de-la-Zouch. Our service operates 07.30 to 18.00, Monday to Sunday, therefore this post requires the candidate to be fully flexible and be able to cover all shifts - there is flexibility in the length of shifts - from 7.5 to 9.5 hours per day. The service also operates on weekends and bank holidays - enhancements are paid when working on these days.
You will need to demonstrate good communication skills and be able to work independently in a variety of community settings. You will need a flexible approach and the ability to work as part of a team and to use your own initiative. The following skills are essential to the role:
- The ability to multi-task.
- Excellent telephone manner and communication skills.
Main duties of the job include but are not limited to:
- General administrative duties.
- Using the electronic patient record system (SystmOne).
- Completing electronic referrals and tasks.
- Assisting where appropriate with data collection for audit purposes.
- Data entry.
- Supporting Core Community Nursing and Therapy staff, Team Leads and clinicians.
- Ordering and monitoring of equipment and stores.
You will need to be a good team player, with excellent organisational and communication skills.
