About the Role

You will work collaboratively with the person, carers, and professionals across health and social care to review care and support plans, promote recovery, and uphold the principles of wellbeing and independence to ensure the person’s outcomes are met. This is a vital role in delivering high‑quality, person‑centred reviews in line with the Care Act 2014 and the Mental Health Act 1983, ensuring that adults with mental health needs receive the support they need to live independently, safely, and with dignity.

About You

  • Be a qualified Social Worker and registered with Social Work England.
  • Have knowledge of social work methodologies, social care policies such as person‑centred approaches and asset‑based social work, and relevant legislation including the Care Act, Mental Health Act, and Mental Capacity Act.
  • Have experience of effective assessment and risk management delivery, safeguarding experience, court work experience, and multi‑agency/multidisciplinary working and/or hospital discharge processes.
  • Ideally have experience of working directly with people in the Mental Health field.
  • Have experience of partnership working with adults to identify and meet need with creative and pragmatic solutions.
  • Be a confident communicator skilled in diplomacy, persuasion, assertiveness and conflict management, able to build effective working relationships and to work proactively both individually and as a team member.
  • Have good prioritisation and time management skills and a valid UK driving licence or access to a vehicle.
  • Speak fluent English, a requirement under Part 7 of the Immigration Act (2016).
