This post is a whole-time Clinical Fellow (middle grade) in Paediatric and Adolescent Rheumatology based at Leicester Royal Infirmary to start as soon as possible. The Department of Paediatric and Adolescent Rheumatology provides clinical services for patients with all types of rheumatic disease. Referrals are received from the primary care, community paediatric team and other departments within the Trust. Joint injections under Entonox are carried out in the day care unit and the GA list is supported by the other Paediatric colleagues. The team works in close relationships with all paediatric sub-specialities including Dermatology, Endocrine, Respiratory, Cardiology, Ophthalmology, Gastroenterology and Nephrology speciality teams. There is a dedicated Paediatric Uveitis service. There is also a dedicated transitional process, attended by an Adult Rheumatology colleague. Patients in the age group 14-18 yrs are reviewed in the transitional clinic and after 18 yrs transitioned to the dedicated Young Adult Rheumatology clinic run by the Adult Rheumatologist.

Main duties of the job

  • The activities in the typical week will include ward rounds, day-care reviews, assessment of inpatient referrals, virtual homecare prescribing, virtual day-care prescribing, response to all clinical queries, telephone consultations (outreach, primary care, other specialties) and joint injections.
  • Participate in the out-patient care of a full range of Paediatric and Adolescent Rheumatology conditions.
  • Participate in the emergency and elective management of children, teenagers and young adults with rheumatology problems admitted to University Hospitals of Leicester NHS Trust.
  • Review paediatric rheumatology patients as required in the paediatric/TYA day care unit.
  • Be responsible for arranging the admission of patients for inpatient and day case investigations and treatment.
  • Be responsible alongside Consultants for Entonox and theatre lists for children/TYA patients undergoing joint injections.
  • Participate in assessment of in-patient referrals.
  • Respond to clinical queries and communicate effectively with outreach, primary care, other specialties and families.
  • Be responsible for virtual prescribing for day care and home care for Paediatric Rheumatology patients.
  • Liaise closely with the specialist nurses.
  • Work closely with the multidisciplinary team ensuring co-ordinated delivery of care.
  • Attend/contribute to MDTs.
  • Participate in teaching, audit and service development.

Governance

  • To work effectively with the CBU clinical quality & safety co-ordinator to identify and reduce risk, incidents and complaints to ensure that the team learn from any such occurrences to improve patient safety.
  • To be responsible for ensuring that consistently high standards of patient care is delivered by example and mentorship.
  • To ensure that patients and families become expert in their own care.

Research

  • To instigate and participate in clinical audits.
  • To support the paediatric research profile.
  • Participate in regular ward teaching of medical students and in weekly case presentations and junior medical staff teaching.
  • The fellow will also be encouraged to participate in the teaching programmes for nursing and paramedical staff.

0900-1700 hrs (Monday-Friday)

  • Fri – AM: Ward review/Clinic/ GA joint injections list | PM: Audit /Research/publications

Note: Job plan subject to change depending on the speciality needs.

Staff

The post does not carry any formal line management responsibility, although there will be an expectation that the post holder will be expected to lead and mentor Junior doctors and other Clinical Staff where appropriate.

Policy

The post holder will be expected to comply with national and local policies DOH and NICE guidance where applicable. The post holder will additionally be expected to be fully engaged with the clinical and managerial teams to develop policies locally where appropriate.

Person Specification

Education / Qualifications

  • GMC registration
  • MRCPCH or equivalent
  • Previous experience at Paediatric Middle Grade level in the UK

Experience

  • Previous experience in Paediatrics, including emergencies and safeguarding
  • APLS (or equivalent)
  • Ability to keep accurate and legible patient records and statistical information
  • Evidence of Leadership experience
  • Understanding management of chronic illnesses
  • Experience in Teaching/ training undergraduates, junior medical staff/nurses

  • Good communication skills, verbal and written
