The predicted salary is between 30000 - 42000 £ per year.
We are looking to recruit a full-time Health and Wellbeing Coach, to work within the West Somerset Primary Care Network, although hours are negotiable between 28-37 per week. The role will provide 1:1 support to people referred to them by team members, group education/coaching workshops and physical activity sessions at community level.
The role of Health and Wellbeing Coach centres on improving patient and community wellbeing – physical, mental, social and emotional – through supporting people to make more informed and conscious choices about their health.
The role enables people to develop the knowledge, skills, and confidence to take opportunities to become active participants in their care. The Health Coaches are based within the Neighbourhood hub in West Somerset, alongside the existing pharmacy, social prescribing and \’Living Better\’ complex care teams. Working in partnership with the PCN and local community they ensure appropriate support is available to individuals, families and carers to improve the quality of their mental and physical health and wellbeing.
The successful candidates will have good communication, negotiation and people management skills and act with compassion, empathy and integrity. Experience or familiarity with person-centred coaching conversations is desirable.
The post holder will work with a diverse range of people and communities and support them to develop their knowledge, skills and confidence to actively look after their own health and wellbeing through positive behaviour change. A compassionate and practical understanding of the barriers facing those from varied social, economic and cultural backgrounds, or living with long term health conditions, including mental health issues, will be important.
The Health and Wellbeing Coaches roles are non-clinical.
This is a full-time role with some flexible working, there will be a requirement for some evening and Saturday availability. A core training package, as well as ongoing development opportunities are integral to the role.
