Part‑time Reablement Co‑ordinator – 21 hours per week for a fixed term of 12 months. The role is based at the Neville Centre on the Leicester General Hospital site and involves working a mixture of morning (7:30 am–4:00 pm) and evening (1:45 pm–10:15 pm) shifts across a rota covering 7 days a week, including weekends, bank holidays and public holidays.
Responsibilities
- Plan and programme all support visits for the Reablement Service across the Leicester area to enable residents to remain independent and avoid unnecessary hospital admissions.
- Attend shift handovers and ensure all outstanding visits are clarified with residents awaiting cover.
- Receive and prioritise phone calls and requests from staff and the public, responding to all within 2 hours.
- Enter care plans and allocate visits using the OneTouch Electronic Rostering System.
- Maintain accurate records of visits, including cancellations and reasons, and produce timely reports for case‑responsible staff.
- Process all incoming referrals to the Reablement Provider Service as part of a multi‑disciplinary team.
Qualifications and Experience
- Strong organisational and IT skills with the ability to prioritise workload under pressure.
- Excellent interpersonal and communication skills, capable of engaging with a diverse population in both verbal and written formats.
- Previous coordination experience in a social‑care setting is desirable.
- Self‑disciplined, flexible, and able to adapt communication style to individual needs.
- Subject to an Enhanced DBS check.
