Live Well Services is a specialist Hub Team based at Braunstone Health and Social Care Centre, which supports the residents of Leicester city to improve their health and wellbeing by offering support with smoking cessation, increasing physical activity, managing alcohol levels, healthy eating, and weight management. This is an exciting opportunity to join our team and help clients through their journey to improve their healthy lifestyle behaviour.
Working with the Hub Manager and other advisors, you’ll provide an efficient information and support service for clients who have been referred. This is a full‑time, permanent position working 37 hours per week from Monday to Friday. You’ll be required to work on a rota basis which includes evenings and weekends. You’ll have flexibility in hybrid working being both office-based at Braunstone Health and Social Care Centre or working remotely from home to suit your needs.
What you’ll be doing:
- You’ll handle incoming referrals, contact clients via telephone to begin the triage process, and respond to email enquiries from the public, existing customers, and professionals such as GP Practices, Cardiac teams and Physiotherapy services.
- You’ll signpost clients to local authority and community teams, as well as book appointments with a range of healthy lifestyle services, some of which operate within the Live Well Service.
- To ensure clients can successfully progress on their lifestyle‑improvement journey, you’ll follow up and monitor your allocated caseload, respond to missed appointments, arrange follow‑up sessions, schedule appointments and make onward referrals where appropriate.
- In addition, you will be trained as a Stop Smoking Advisor, enabling you to deliver evidence‑based behavioural support, provide nicotine‑replacement guidance, and work directly with clients who wish to quit smoking as part of their wider health‑improvement goals.
What you’ll need:
- With experience of working in a pressurised frontline service, you’ll have experience of providing a telephone and/or face-to-face service in a busy customer service environment.
- Using excellent verbal and written communication skills, you’ll provide clear and concise advice to clients and members of the public and complete all electronic records accurately and in a timely manner.
- Being highly organised, you’ll be able to work under pressure and deal with customer enquiries until they are completed.
- With tact and diplomacy, you’ll be able to sensitively deal with customers who may be distressed.
