About the role

To apply for this secondment position you must obtain permission from your current line manager prior to submitting an application. We are looking for an experienced nursing lead who is passionate about improving patient experience, driving system‑wide change, and enabling people to safely return home sooner.

Key responsibilities

  • Champion the Home First philosophy by ensuring timely assessment, coordinated discharge planning, and appropriate support for patients.
  • Provide daily operational leadership across acute, community and intermediate care settings, collaborating with nursing, therapy, social care and third‑sector partners to reduce length of stay and maximise time spent at home.
  • Lead and develop the following teams: Discharge Liaison, Porth Preseli Clinical Streaming Hub, Hospital @ Home, Acute Response, Home Support and Community Falls services.
  • Coordinate discharge planning, apply specialist knowledge of commissioning care and implement local and national improvement initiatives; drive pathway development to enhance flow.
  • Line‑manage the teams listed above and act as a credible leader across the wider Health and Social Care system.

Qualifications & experience

  • Substantial experience across acute and community care, with well‑developed clinical expertise.
  • A passion for service improvement and demonstrated ability to influence, negotiate and collaborate across the healthcare system.
  • Strong operational coordination skills focused on patient flow, timely assessment and discharge.

Desirable

The ability to speak Welsh is desirable, but English and/or Welsh speakers are equally welcome to apply.

Additional information

Interviews will be held on 11 March 2026. The Health Board offers support and learning opportunities to help staff meet Welsh language requirements.
