Job Summary
The Community Mental Health Team is a multi‑disciplinary group that undertakes comprehensive assessment, care coordination under the Mental Health Measure (Wales) 2010, and works closely with other statutory services, third sector partners and local providers to deliver tailored care. The role involves traveling between sites and supporting patients through recovery.
Key Responsibilities
- Conduct initial comprehensive assessments of service users.
- Perform risk assessment and management within the Community Treatment Plan (CTP) framework.
- Provide crisis intervention and home‑treatment support.
- Coordinate care in line with Mental Health Measure (Wales) 2010.
- Maintain accurate documentation and collaborate with multi‑disciplinary teams.
- Prioritise workload and manage time effectively.
- Travel regularly between sites to deliver care.
Qualifications and Knowledge
- Registered Mental Nurse (RMN)
- Post‑basic education to postgraduate diploma level
- Post‑registration experience in an acute mental health setting
Experience
- Wide range of experience in a service‑related setting
- Demonstrated achievement in developing care standards, procedures and guidelines
- Effective team‑work skills
- Proficiency in IT systems relevant to nursing practice
Desirable
- Relevant degrees (post‑graduate in mental health or related discipline)
