An exciting opportunity has become available for a Band 6 Community Mental Health Nurse who has the relevant qualifications and experience in Acute Mental health. The team is based in Bro Cerwyn, Fishguard Road, Haverfordwest, Pembrokeshire.

The Community Mental Health Team is a multi-disciplinary team that undertakes various roles and duties including initial comprehensive assessment of people accessing the service. We offer care coordination under the Mental Health Measure (Wales) 2010 to support people through their recovery. The Community Mental Health Team works closely with the wider mental health services within Hywel Dda, other statutory services and third sector services to ensure the people accessing mental health services are provided with the most appropriate care for their needs. The ability to travel between sites in a timely manner is essential.

Main Duties

Applicants will have significant experience working in an acute adult mental health setting and will require a sound knowledge of current issues pertaining to mental health, crisis intervention and home treatment. Comprehensive Assessment, Risk Assessment and Management skills within the context of CTP are essential, as are excellent interpersonal and communication skills. The ability to manage time and prioritise workload is fundamental to effective service delivery. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

  • 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers

  • RMN
  • Post-basic education in the area of practice to postgraduate diploma level
  • Post registration experience
