Overview

This post comes during a period of Service Transformation in response to the changing needs of people with a learning disability and the national agenda for improvement. If successful, you will play a vital part within this exciting period of change, by contributing to the design of a new service model providing flexible & responsive services. We are looking for a suitably qualified, enthusiastic & motivated nurse with current NMC registration to join our Learning Disability Community Team based in Carmarthenshire. You will be expected to work proactively & creatively as part of a multidisciplinary team to deliver high quality, evidence based nursing care to adults & young people with a learning disability in partnership with them & their families/carers. You will have the opportunity to work closely with other colleagues in the multi-disciplinary team, and will provide specialist advice, support & training to those on your caseload, their families & carers, and to your colleagues across the Health board. You will also be supported to develop your own clinical competency profile & to develop skills in specialist areas of practice. Applicants should have postgraduate experience within a healthcare setting, have a broad knowledge of clinical specialties, with evidence of Continuing Professional Development. We offer regular supervision, professional development & access to peer Nursing support.

You Will Need To Have continuing professional accountability and responsibility for the provision of specialist health care, treatment and advice for a defined caseload of clients who have learning disabilities with complex health needs, and their carers, in the community. Liaise and work in partnership with other agencies to deliver a flexible and responsive service which meets the need of the local population of people with a learning disability and the needs of carers. Provide specialist advice, support and training on a range of health issues to other agencies who are responsible for the provision of services/care to people with learning disabilities with the aim of reducing health inequalities for people with a learning disability. Develop, maintain and be responsible for own knowledge / skills base evidencing professional development through a personal portfolio and participation in annual PADR. Assess, supervise and appraise designated unqualified health care workers and student nurses. Play an active role in contributing to the development and improvement of the Learning Disability Service in accordance with the vision, aims and objectives of the service.

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services. We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.

  • Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
  • Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
  • 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
  • Numerous mental health and learning disabilities services

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
