We have an exciting opportunity for a Band 5 Community Nurse to join our Community Nursing Team based in Milford Haven, Pembrokeshire. This role involves supporting the management of a delegated caseload within the Community Nursing Team, providing high quality and complex care management to patients in their own homes, with guidance and support from an experienced District Nurse Team Leader. The post holder will ensure high standards of care are delivered to patients and their families/carers within the home environment, liaising with staff, patients/carers and other agencies/organisations as required to ensure appropriate delegation of duties and risk management, whilst ensuring effective communication between those involved in the care package.
Main duties include:
- Pembrokeshire's Community Nursing Teams have a wide-ranging nursing skill mix and pride themselves on supporting professional development and improving high standards of community practice in line with current health policy.
- This is a permanent position of 30 hours per week worked over a 7-day period including weekends and bank holidays.
- The post holder will need the ability to travel between sites in a timely manner.
- The ability to speak Welsh is desirable for this post: English and/or Welsh speakers are equally welcome to apply.

Essential – Qualifications and Knowledge- Level 1 Registered Nurse (Adult)
- Current NMC Registration
- Evidence of personal continuing development, statutory and mandatory training
- Awareness of Health Board policies, procedures and guidelines
- Ability to relate theory to practice
- Understanding of primary health care working
- Post-registration experience, in acute services
- Teaching and assessing
