Main duties of the job

- Deliver high-quality, compassionate care and specialist advice to patients, families, and caregivers.
- Communicate sensitively and effectively with patients, families, and multidisciplinary teams.
- Support and educate colleagues to ensure holistic care planning, including advance care planning and DNACPR discussions.
- Identify patients with palliative care needs early to prevent unnecessary delays in treatment
- Manage a defined caseload under the guidance of the Consultant, working closely with Clinical Nurse Specialists, doctors, therapists, and the wider community MDT.
- Contribute to service improvement, education, audit, and research initiatives.
What We\’re Looking For:
- A registered nurse with significant experience in palliative care.
- Excellent communication skills and emotional intelligence.
- A passion for delivering proactive, person-centred care.
- Ability to work both independently and as part of a multidisciplinary team across the community.
