Responsibilities:
The post holder will work as part of a small specialist CLA Health Team and be professionally responsible for a defined cohort of children looked after and vulnerable children placed away from home.
Co‑ordinating and quality‑assuring the completion of specialist Health Assessments by other Health Professionals and undertaking statutory health assessments for children aged 0‑18 years.
Utilizing specialist knowledge and skills to develop individual specialist health care plans and deliver interventions to meet the health needs of children looked after and vulnerable children and young people, improving their health outcomes.
Working with care leavers and contributing to the development and implementation of specialist programmes of care as required.
Providing specialist knowledge, support and guidance to Social Services, Foster Carers, Education and other relevant agencies and health professionals, on the health and wellbeing of Children Looked After, including providing clinical supervision and delivering specialist training programmes.
Providing specialist health advice and knowledge to the multi‑agency care assessment and operational planning of services for CLA and Care Leavers, working closely with Local Authorities Social Services Departments.
Interpreting complex and sensitive health information and reaching conclusions based on clinical judgement, research evidence and management of risk to promote and improve individual health outcomes.
Co‑ordinating the completion of statutory Initial and Review Health Assessments for CLA, identifying the most appropriate health professional to undertake the assessment (e.g., Health Visitors, School Nurses, Paediatricians and GPs).
