An exciting opportunity has arisen for recruitment to the Carmarthenshire community nursing service. The post requires an enthusiastic and proactive nurse to provide a high standard of patient care, with leadership, team and partnership working. Applicants should have substantial community experience with a postgraduate qualification in a relevant course (or be working towards it).
You will work without direct supervision and be accountable for delivering high standard, cost-effective nursing care in the community. You will lead on promoting high-quality care within the team and supporting team members to achieve excellent patient care. The post requires excellent communication, a flexible approach, good time management and the ability to travel between sites. Strong IT skills are essential.
Main duties:- Combine excellent clinical practice with day-to-day shift coordination and undertake leadership and management responsibilities of the Carmarthenshire Continuing Care Team.
- Provide supervisory management and strong clinical leadership; recognise pressures on staff and develop mechanisms to support and develop staff in caring for people with Continuing Health Care and Primary Health needs in a complex, pressurised community environment.
- Work with the community nursing team, Patient Flow and the wider community MDT to support junior colleagues in delivering direct care to meet continuing health care needs for individuals, groups and the wider community.
Welcoming Welsh and English speakers; bilingualism is encouraged. The ability to speak Welsh is desirable. Interviews scheduled for 19 February 2026.
