Band 6 (Unscheduled Care Practitioner) role based in Aberystwyth, Ceredigion. The CRHT team provides intensive support and home treatment for people experiencing mental health related crisis and offers alternatives to in‑patient care. Service operates a 24/7 shift system and requires travel between sites.
Responsibilities
- Assess and home treat people who attend general hospital and inpatients in hospital settings.
- Manage risk assessment, crisis intervention and home treatment within the context of CTP.
- Prioritise workload, manage time and communicate effectively with service users, families/carers and multi-disciplinary team.
- Travel between sites in a timely manner.
Qualifications & Knowledge
- RMN registration and postgraduate diploma level education.
- Post registration experience in an acute adult mental health setting.
Experience & Skills
- Comprehensive assessment, risk assessment and management skills.
- Excellent interpersonal and communication skills.
- Demonstrable IT skills and effective team working.
Desirable
- Welsh speaking (Level 1). English speakers welcomed.
- Relevant degrees, clinical audit and research experience.
