Anexciting opportunity for both full and part time hours has arisen to join theLeeds Childrens Hospital acute patient flow co-ordinator team.

Weare looking for an enthusiastic individual to take responsibility andactively manage patient flows across the Childrens hospital by implementingand maintaining processes that safely expedite patient placement anddischarge.

Thejobholder is responsible for the timely and appropriate placement of patientsrequiring admission to the bed base and for the escalation procedure from theEmergency Department and the CAT unit whilst working closely with the matronand service manager teams to admit elective patients.

Therole will give you an insight into the wider organization and the issues anddrivers which shape current paediatric services. You will develop advancedskills in identifying risks and solving complex issues and gain valuableexperience in balancing staff availability and skills against the needs ofpatients across the service.

Main duties of the job

Thesuccessful candidate will facilitate patient flow within the Childrenshospital. In addition, they will support the clinical nursing teams in theirday to day working. They will work closely with all areas of the Childrenshospital, the discharge team, paediatric A&E and the CAT unit to ensurepatients are admitted in a timely manner.

Inaddition, they will report into the wider trust working closely with the CSMteam whilst also supporting the Childrens hospital with some allocatednon-clinical time to look at quality improvement and waste reductionprojects.

Job responsibilities

Thejobholder will provide highly specialised nursing advice specific to thepatient group or speciality.

Thepost holder will be will be responsible for flow of patients into and outfrom the childrens hospital consideringthe care needs of eachindividual child or young person and the development, implementation andevaluation of programmes of care Practising as an independent specialistclinical practitioner providing patient-centred and evidence-based clinicalcare, within an agreed scope of competence and practice. The jobholder isresponsible for the management of the flow of children in and out of thechildrens hospital.

Thejob holder will contribute to the development of the speciality service aswell as contributing to the strategic direction of childrens nursing withinthe Trust.

Thejobholder will ensure cost effective use of resources. The job holder willsupport the development of interdisciplinary and whole service working. As a role model they will promote thesharing of good practice and delivery of high quality, effective andefficient care. The job holder will provide training and education to staffgroups, patients and families/carers.

Thepost holder will work in partnership with the multi-disciplinary team (MDT)to implement and improve the patient pathway and will be expected to developnew skills to further enhance the quality of the service delivered. Whereappropriate, the role will include the day to day leadership and management.

Qualifications

  • Registered Children\โ€™s Nurse
  • Leadership Qualification
  • Formal Teaching Qualification
  • First degree and/or Higher Academic Study (Master\โ€™s Degree)

Experience

  • Significant experience at sister/charge nurse
  • Insight into current issues in nursing/professional issues
  • Knowledge of Standards of Practice
