We have an opportunity for a band 6 Discharge Facilitator Specialist Nurse within Trauma and Related Services Department (TRS).
Main duties
- You will be implementing, coordinating and facilitating safe and timely discharges for patients within the TRS bed base.
- You will follow their pathway throughout their journey through all of our services from the initial admission through to discharge.
- You will be the main contact for families and carers supporting them during the discharge process.
Responsibilities
- You will be responsible for leading a team of band 3 discharge coordinators and act as senior support for TRS in the absence of the band 7 discharge lead.
- The role requires attendance at MDT daily where you will alongside the multidisciplinary team identify same‑day and future discharges, any delays in service that will prevent a smooth transition from hospital to home.
- The role requires you to remain in close contact with the operational matron by attending twice daily patient flow meetings to provide discharge numbers and escalations to support early discharges.
- The discharge team operates a seven‑day service and you will be expected to work some weekends and bank holidays.
- We will provide local training for the successful candidate and you must be a highly developed communicator as this is essential to the role.

- Registered Nurse, Physiotherapist or Occupational Therapist
Experience
- Significant post registration experience at Band 5 or equivalent
